Mortgage Application Checklist 

• If applicable, copies of W-2s from the last two years for all borrowers

• If applicable, copies of paystubs for all borrowers covering the last 30 days and reporting year to date earnings.

• If any borrower is self-employed, complete copies of personal federal income tax returns with all schedules for the last two years and if applicable, a year to date income and expense statement for the business through the end of the most recent calendar quarter.

• Copies of documentation to support other income that  is received on a regular basis, such as retirement or pension benefits, social security benefits, disability income, alimony, child support, trust income, interest/dividend income, etc.

• Copies of the most recent bank statement for all bank accounts (other than those with CGSB).

• Copies of most recent statements for all investment accounts, including retirement accounts.

• A copy of the most recent real estate tax bill on the property and all other real estate you own.

• A copy of your homeowners insurance binder with the annual premium.

• A copy of the existing deed on the property.

• If applicable, copies of the condominium by-laws and the blanket insurance policy on the condominium complex with minimum coverage of $1.0 million.

• If applicable, the name and address of the manager of the condominium so we may mail a condominium questionnaire to his/her attention.
